Hi everyone, we're exploring a new capability for Reliance eHealth Collaborative that could help your organizations make smarter participation decisions around the CMS ACCESS (Advancing Chronic Care with Effective, Scalable Solutions) Model.
Here's the opportunity. We're considering offering data readiness audits that would help your clinical teams assess whether becoming an ACCESS provider makes sense for your organization - or whether referring to a partner would be a better path. Essentially, we'd help you understand your data completeness, quality, and chronic care cohorts against ACCESS Model requirements before you commit resources.
Why this matters. The ACCESS Model has specific data and reporting requirements. Most organizations want to know upfront: "Do we have the infrastructure and data quality to succeed as an ACCESS provider?" Rather than finding out mid-engagement, we could help you answer that question now and avoid costly missteps.
So here's what I'm trying to understand from you:
First, is your organization considering participating in the CMS ACCESS Model, either as a direct provider or through a partner? Second, if you're evaluating this, what are your biggest unknowns right now - is it data readiness, ROI projections, patient cohort definitions, or something else? And third, would a readiness audit from us - including recommendations on whether to go direct or partner - actually influence your decision-making?
Bonus question: If we built this, would you want it as a one-time audit, or ongoing monitoring and reporting as you prepare for implementation?
